Medicare plans are sold by county, so the disclaimer above depends on where you live. Enter your ZIP code — the page looks up your county on the web and shows two separate views: what we represent (only the organizations Emerald Life & Health is contracted with — that view fills the disclaimer) and everything available in your area (all organizations offering products in your county, including ones we don't represent — information only, so you can see the full picture and check Medicare.gov for all of your options).

Medicare questions? Start with one call — on your terms.

Tell us you're okay to be contacted, and we'll take it from there. A customer service representative from Emerald Life & Health will call first to pre-qualify you — a few quick questions so your time isn't wasted — and can then transfer you to a licensed insurance agent who can walk through the Medicare Advantage and Part D plans we offer and, if you choose, enroll you.

  • No obligation. Asking for a call does not enroll you in anything.
  • Your current coverage is safe. A call does not affect your current or future Medicare enrollment, and nothing enrolls you automatically.
  • You choose the topics. We only discuss the product types you select in the form — nothing beyond that scope.
  • Nothing to buy on this page. There is no plan shopping or enrollment here. If you decide to enroll, your licensed agent does that with you on the call, through SunFire.

Request a call

This form is your Permission to Contact. It also records the Scope of Appointment — the product types you agree to discuss — before any personal marketing appointment takes place.

1 · Your information

Please enter your first name.

Please enter your last name.

Please enter a phone number with at least 10 digits.

Please enter a valid 5-digit ZIP code — plan availability depends on where you live.

2 · What we may discuss — your scope

Please select at least one topic — we can only discuss what you select.

Your scope is good for 12 months from today. If you'd like to talk about a different product type later, we'll record a new scope with you first. Products outside the topics above won't be discussed on this call.

3 · Please read — then give your permission
Scope of Appointment statement By submitting this form, you agree to be contacted about the product types you selected above. Please note, the person who will discuss the products is either employed or contracted by a Medicare plan. They do not work directly for the federal government. This individual may also be paid based on your enrollment in a plan. Submitting this form does NOT obligate you to enroll in a plan, affect your current or future Medicare enrollment, or automatically enroll you in the plan(s) discussed.
How your information is used Your information will be provided to a licensed insurance agent. You may be contacted by a licensed insurance agent. In practice, a customer service representative of Emerald Life & Health usually calls first to pre-qualify you, and you may then be transferred to a licensed insurance agent who can enroll you in a new plan. Plan benefits are discussed — and any enrollment completed — only with the licensed agent, through SunFire, the quoting and enrollment platform our agents use. We will not share your personal information with another marketing organization without your separate written consent naming that organization.

Please check the Permission to Contact box so we have your permission to call.

Please check this box to confirm you understand how the call works.

Permission recorded

Thank you — your Permission to Contact is on record.

A customer service representative from Emerald Life & Health will call at your preferred time to pre-qualify you, and can then transfer you to a licensed insurance agent. Keep this record for your files — it shows exactly what you agreed to, and when.

How the call works

You don't have to reach a licensed agent on the first ring — and you stay in control at every step.

1

A customer service rep calls first

An Emerald Life & Health customer service representative calls at your preferred time to pre-qualify you: confirming your ZIP code, the topics you selected, and basic details like your doctors and prescriptions, so the licensed agent's time with you is well spent. This rep does not discuss plan benefits or enroll you.

2

You're transferred to a licensed agent

When you're ready, the representative transfers you to a licensed insurance agent. Before any benefits are discussed, the agent reads the required disclaimer for the plans we represent in your area. You can end the call at any point — there is no obligation to enroll.

3

The agent compares plans in SunFire

Your licensed agent uses SunFire — the Medicare quoting and enrollment platform — to compare the plans we offer in your area against your doctors and prescriptions, and to complete an enrollment only if you choose one. This page itself offers no plan shopping and takes no enrollment.

Common questions

Does requesting a call enroll me in a plan or change my current coverage?

No. Submitting the form does NOT obligate you to enroll in a plan, affect your current or future Medicare enrollment, or automatically enroll you in any plan. Enrollment only happens if you choose a plan with a licensed agent and explicitly agree to it on that call.

Why does a customer service representative call before the licensed agent?

The representative pre-qualifies you — confirming your contact details, service area, and the topics you chose — so the licensed agent can prepare. Your information will be provided to a licensed insurance agent, and you may be transferred to that agent on the same call. Plan benefits and enrollment are handled only by the licensed agent.

Do you offer every Medicare plan in my area?

We do not offer every plan available in your area. Currently we represent [insert number of organizations] organizations which offer [insert number of plans] products in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.

Enter your ZIP code at the top of this page and those counts update to your county automatically.

Which companies' plans can you actually enroll me in?

Only the organizations we represent: UnitedHealthcare (UHC), Humana, Devoted Health, Aetna, and Wellcare. The area checker at the top of this page also shows everything available in your county — including organizations we don't represent — so you can see the full picture. We can't discuss or enroll you in those other organizations' products; for information on all of your options, contact Medicare.gov or 1-800-MEDICARE.

When can I enroll?

Enrollment in a plan may be limited to certain times of the year unless you qualify for a Special Enrollment Period or you are in your Medicare Initial Enrollment Period. A licensed agent can check which enrollment periods apply to you.

How do I stop future calls or texts?

Tell the caller at any time, or reply STOP to any text message. Revoking your permission stops marketing contact from Emerald Life & Health under this request; it does not affect any plan you already have.

Required disclosures

CMS verbatim · TPMO disclaimer

We do not offer every plan available in your area. Currently we represent [insert number of organizations] organizations which offer [insert number of plans] products in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.

Standardized content under 42 CFR 422.2267(e)(41) / 423.2267(e)(41), current for CY2027 marketing (from October 1, 2026). The counts in this disclaimer are only the organizations and products Emerald Life & Health offers through SunFire in your service area — its contracted carriers: UnitedHealthcare (UHC), Humana, Devoted Health, Aetna, and Wellcare. Read aloud on sales calls before any benefits are discussed, and displayed prominently here as CMS requires for TPMO websites.

How the counts get here: when you enter a ZIP code, this page resolves it to a county using public web lookups (ZIP-to-place data and federal county data), then fills the counts in from a county table built from the CMS CY2027 Medicare plan landscape file (revision 202610, data updated October 18, 2026), refreshed each plan year. That table powers two separate views: “What we represent” — filtered to the five contracted organizations above, and the only view used in this disclaimer — and “Everything available in your area” — every organization and product in the county, shown for information only alongside the direction to Medicare.gov / 1-800-MEDICARE for all of your options. Emerald Life & Health does not sell or enroll anyone in products from organizations it does not represent. If a ZIP cannot be resolved, the disclaimer falls back to the agency's default counts rather than guessing.

Federal contracting statement

Participating sales agencies represent Medicare Advantage [HMO, PPO and PFFS] organizations [and stand-alone PDP prescription drug plans] that are contracted with Medicare. Enrollment depends on the plan's contract renewal.

CMS requires this statement to convey the plan's Medicare contract and renewal contingency (42 CFR 422.2267(e)(32)); the exact wording is the carriers' template, confirmed with each carrier at filing.

Not a government site

Emerald Life & Health is not connected with or endorsed by the U.S. government or the federal Medicare program.

Before this page goes live

CMS does not approve agency pages directly. This page's wording must clear the normal route first:

  • TPMO lookup table built from CMS data — done — carrier list confirmed (UHC, Humana, Devoted, Aetna, Wellcare); the county counts come from the CMS CY2027 plan landscape file (revision 202610, data updated October 18, 2026) — represented counts from those five organizations only, full-area counts from every organization in the county, across all 3,221 counties.
  • Carrier pre-review & HPMS filing — each contracted carrier reviews this material; it is filed in HPMS (directly or through the FMO) and each carrier opts in before use (45-day review, 10 days for model/standardized-based material, or File & Use where designated).
  • Carrier overlays checked — each carrier's own Federal Contracting Statement form, SOA form version, and any stricter rules (for example, longer call-recording retention) applied.
